Thank you for your interest in this survey. I appreciate your participation and look forward to reading about your experiences with the Oregon Trail Pageants.
The results of this survey will be used solely for the purpose of researching the Oregon Trail Pageants and local Eugene theatre history, and will not be distributed other than in the context of this project. A full description of this proposal and resources are available by contacting the researcher.
This survey consists of 16 questions. The time taken to complete the survey is dependent upon the information provided by the individual responding. Please answer as many of the questions that you feel you can. If one does not seem applicable to you, skip it and respond to the next one that applies. You may want to type your responses in another program such as MS Word and then copy or paste them into the survey form. Conversely, you could copy the questions from the survey, paste them into a Word document, and then send the document to the researcher via email or the postal service. (In either circumstance do be aware of formatting changes that may occur from switching formats.)
Your participation is voluntary. Your decision whether or not to participate will not affect your relationship with the researcher or others in this study. Participants, with proper notification to the researcher, may withdraw participation from the study at any time.
By submitting the survey form you are giving your consent for any of the information provided (except for any personal and contact information) to be used for research purposes by Julie L. Voelker-Morris. Information may be utilized towards future publications and presentations, and any direct quotations will be properly attributed to the author. Participant contact information will never be shared, sold or distributed. As needed, follow-up interviews may be requested.
Your submission of the survey form indicates that:
